Care Coordination is Concierge Care!

A key-but often overlooked-service of Partnership Health Centers is care coordination. Because communications among medical providers treating the same patient may be limited, even the best providers may not know about treatment plans from other doctors and healthcare professionals which affect their patients. This lack of information can prevent good health outcomes. Worse, it can endanger those who are seeing multiple doctors and taking multiple medications.

National standards call for health plan administrators to provide just such Personalized Care Coordination Services for their members. Unfortunately, this is usually done only through remote contact by insurance carrier staff who never have personal contact with the patient. Because Partnership Health Centers is dedicated only to its members and not open to the public, its care coordination staff are always available for personal visits!

Best Possible Care

Nurse Care Coordinators will assist you with chronic health conditions such as diabetes, congestive heart failure, lung diseases such as asthma or COPD, depression and anxiety, and chronic pain, which often require involving multiple specialties and services. The Care Coordination service has you covered! Our sole goal is to work with you to arrange the best possible care team with providers of your choosing.

There are three, very important aspects of Partnership Health Centers' care coordination process to keep in mind:

  1. Your personal health information is NOT accessible by your employer. It is managed by the same, HIPAA compliant, patient confidentiality requirements in place for all medical services.
  2. The purpose is not to change doctors and specialists you have chosen for yourself. We encourage you to keep them. Our Care Coordination team works to facilitate collaboration among them, so unintentional mistakes may be avoided.
  3. There are no additional costs for this Partnership Health Centers service! This is a service offered by our professional staff to all participants in your health insurance plan.
Although collaboration with our Care Coordination team is voluntary, they will reach out to you when our system information indicates critical information needs to be shared.
