Frank Mazzotta, D.O.

Dr. Frank Mazzotta D.O. is the Medical Director of Partnership Health Center Lycoming County (PHCLC). In addition to being a graduate of The Philadelphia College of Osteopathic Medicine with a fellowship in Family Practice, he is also Board Certified in both Hospice and Palliative Medicine. Prior to joining PHCLC, Dr. Mazzotta maintained a private Family Practice over the last 20 years and also worked for Vitas Healthcare.

Dr. Mazzotta is a long-term resident of Philadelphia but will be living in Lycoming County during the week. He became interested in joining the local community once he learned of PHC's history of providing easy access to care, which he regards as vital to good health. Dr. Mazzotta had been working in a variety of jobs and settings over the years and cited the list of available services in Partnership Health Centers as another reason he was motivated to join the organization. In addition to same-day appointment scheduling for medical visits, provided by both an MD and Nurse Practitioners, there is also Physical Therapy (PT), X-Ray, an on-site pharmacy (opening soon), laboratory services and Behavioral Health services with a Masters level clinician - and there are never any co-pays or deductibles.

Our first Pennsylvania location follows the successful model found in the other six (6) locations in central New Jersey. The focus of our efforts is to help individuals treat one mind and body in a single healthcare "home" ...a one-stop-shop. Over many years, this has helped numerous families and individuals gain access to care they may have otherwise forgone due to financial issues or the inability to find streamlined medical care.

Dr. Mazzotta recounted the situation of a patient recently seen at PHCLC who expressed that they had been plagued by ongoing shoulder pain for over a year. They had not been able to adequately address their medical problem, primarily as a result of not being able to afford the ongoing co-pays that were expected at each visit. Dr. Mazzotta was happy to report that this patient is now receiving the regular and ongoing treatments they need without worrying about the financial burden that prevented them from seeking care for so long. He reported that seeing the smile on his patient's face made his day, knowing that they will now be OK as they are no longer faced with the financial barriers previously in place.

Michele Rook, LPN

Michele Rook, LPN began her nursing career at Northern Tier Career Center and has been a Licensed Practical Nurse for the past 20 years. She has extensive experience in skilled nursing, family practice, and home health, having worked both directly with patients and in supervisory roles within home healthcare.

She joined Partnership Health Centers nearly a year ago as an LPN and was promoted to Nurse Manager after just eight months. Michele loves working at Partnership Health Center Lycoming County, where she values the compassionate and dedicated team that feels like family. One of her favorite aspects of her role is working in the lab, where she excels in doing bloodwork and getting to know patients.

Outside of work, Michele enjoys spending time with her husband, Mike, her daughter, Logan, and her grandchildren, Jameson and Rowen. She also holds a special place in her heart for her son, Nathan, who is her angel in heaven. In her free time, she loves reading, baking, and taking her dogs for walks.
